I hereby give permission to Old School Intergenerational Projects (Hereafter, referred to as “Old School”) to take photographs and video recordings of me, and any specified family members listed on the registration form for use in printed, web image, or video formats for advertising and educational purposes.

The images may be edited at Old School’s discretion, and I expressly release Old School, your officers, employees, directors, sponsors, and volunteers from all claims which I have or may have for invasion of privacy, defamation, or any other cause of action arising out of exhibition or distribution of any materials in which myself and/or specified family appear.

 

By signing this document, I represent and warrant that I have the legal authority of the person whose name appears next to mine below, and that we shall both(all) be bounded by this agreement.
